The terms get used interchangeably, and they should not be. If you are booking a smoothing treatment, knowing which one you are actually getting matters more than most salons let on.
What each one does
A keratin treatment coats the hair. It fills gaps in the cuticle, seals the surface, and the result is smoother, shinier hair with reduced frizz. Your curl pattern softens but does not disappear. If you have wavy or loosely curly hair and you want it to behave, this is usually the right choice.

Progressive smoothing goes further. It relaxes the internal structure of the hair rather than only coating it, and the result is straighter. On tightly curled hair the difference is dramatic. On fine hair it can be too much.
The practical distinction: keratin makes your hair easier to style. Progressive smoothing changes how it grows out until it does.
The part you should ask about
Many smoothing treatments contain formaldehyde or chemicals that release it when heated. This is not a fringe concern. Research has linked these products to increased risks of several hormone-related cancers and to uterine fibroids, and salon workers face repeated exposure because they are in the room for every service.
The FDA has been working on a proposed rule to prohibit formaldehyde and formaldehyde-releasing substances in hair smoothing and straightening products. It is worth being accurate about where that stands: the rule has been delayed repeatedly since 2023 and has not been finalized. As of now these products remain legal and widely sold.
So the responsibility sits with the salon and with you.
Ask what is in the product before you book. A salon that cannot answer, or that gets defensive, is telling you something. Formaldehyde-free options exist and have improved considerably, though they generally deliver less dramatic straightening and need more frequent reapplication. That is an honest tradeoff, not a downside to hide.
Ventilation matters too. If the room fills with fumes that sting your eyes, that is not normal and it is not something to sit through.
Who each suits
Keratin works well if you have frizz, humidity problems, or waves you want to soften rather than remove. It typically lasts two to four months.
Progressive smoothing suits tighter curl patterns and anyone who wants genuinely straight hair without daily heat styling. It lasts longer but the grow-out line is more visible, and you are committing to a longer relationship with the treatment.
Neither is a good idea on hair that is already damaged or heavily bleached. If your hair is fragile, the honest answer is to repair first and smooth later.
